Subject: Application for Section .1700 General Pernut for end bent stabilization on Onslow Bridge
#54 over the New River on SR 1231 Banks Bridge Road.

Dear Mr. Lane:
The Narth Carolina Department of Transportation (NCDOT) is proposing to stabilize the northern
end bent of Bridge No. 54 in Onslow County. This project will address erosion at the northern bank
of the bridge structure (end bent 2). The bridge is located on SR 1231 Banks Bridge Road, 0.19 mile
north of SR 1229 (Gregory Fork Road).
Please fmd enclosed the completed DCM MP-1 and DCM MP-2 applications detailing NCDOT's
proposal under the Section .1700 General Permit. NCDOT is also providing a courtesy copy of this
request to the US Army Corps of Engineers (IJSACE) and the Department of Environmental Quality,
Division of Water Resources (DEQ, DWR) to notify them of NCDOT's intention to perform this
work under Nationwide Pernut 3 and Water Quality General Certification No. 4085.
In addition to the completed DCM MP-1 and DCM MP-2 please find the following attachments:

The northern end bent area will be stabilized with fiil dirt and Class 2 grarute riprap. The project
will permanently impact 681 sq. ft. of open water, and temporarily impact 222 sq. ft. for dewatering
measures. The permanent impacts will return the bank to the approximate previous extent as shown
in photos from a 2016 bridge report. All work will be within the existing NCDOT R/W. NCDOT
