Application for 401 Water Quality Certification for the replacement of culvert as
part of the road widening improvements to SR 1686 (Shattalon Drive) Forsyth
County, North Carolina. City Project No. 200502.
Dear Sir or Madam:
In December 2008, the City of Winston-Salem submitted Pre-construction Notification
for replacing an existing pipe culvert. This plan revision is in response to phone
conversation with Amy Euliss with the NC-DWQ Winston-Salem regional office.
Please find the enclosed copies of the Permit drawing, design plan sheet and construction
detail for the above referenced project. The plan revision shows the pipe culvert outlet
shifted and rotated to more align with stream.

PROJECT DESCRIPTION
Impacts to waters of the United States
The project is located in the Yadkin River Basin in Forsyth County. Water resources in
the area include Mill Creek and Bethabra Branch. Bethabra Branch is a perennial stream
that flows north to south within the project boundaries. Bethabra Branch is a tributary to
Mill Creek and does not have an assigned Stream Index Number by the N.C. Division of
Water Quality.
Neither High Quality Waters (HQW), Water Supplies (WS-I or WS-II) occur within 1.0
mile of the project area.
Permanent Impacts
There will be a total of 117 linear feet (LF) of permanent stream impacts to Bethabra
Branch (BB). These impacts are associated with the removal of 40 LF of an existing 12'
by 8' arch pipe culvert and placement of an 89 LF of 15' by 9' arch pipe culvert where
BB crosses the roadway. Of the 117 linear feet, 50 linear feet will be due to the
replacement of the existing culvert and placement of fill to construct the culvert. The
remaining 67 LF of permanent impacts will result from Class I rip rap on the stream
banks at the pipe inlet and outlet.
Temporary Impacts
A total of 25 linear feet (LF) of temporary stream impacts will occur to Bethabra Branch.
These impacts are associated with the placement of the new pipe culvert. A temporary
impervious dike will be installed at the in-flow and out-flow sides of the pipe culvert for
the purpose of diverting stream flow around or through the construction site. The outlet
channel will be benched to reduce channel erosion for high flows.
Proiect Schedule
The project is scheduled for letting in May of 2009 and date of construction availability
of July 2009. It is anticipated that the culvert construction will be completed during the
summer while school systems are on summer break.
