Subject: Sampson 148 Permit Drawing Revisions
Brad & Joanne,
Please find attached revised permit drawings for Bridge 148 over Mill Creek on SR 1434
(Huntley School Rd) in Sampson County. WBS # 17BP.3.R.53. During the
preconstruction meeting it was noted that impacts associated with the stormwater pipe
replacement at �Sta. 20+00 LT were omitted from the original submittal. This revision
includes an additional 23 If of bank stabilization and <0.01 acres of surface water
impacts for dewatering BMP's at the outlet of the new pipe. This project was processed
under the LIBR program and we received e-mail confirmations from DWR on 2/16/17
and USACE 3/17/17 that the project met the terms and conditions of Nationwide 3 and
13, as well as the corresponding WQC's 4085 and 4087. The PJD was assigned AI #
SAW-2017-00414.
Please provide an e-mail confirmation that you have received the revised drawings and
that the project still meets the conditions and terms of your respective permits.
