Subject: LIBR Application for Section 404 Nationwide Permit 3 and 13 for proposed replacement of
Bridge No. 148 over Mill Swamp on SR 1434 (Huntley School Rd.) in Sampson County. WBS
No. 17BP.3.R.53
Dear Mr. Shaver:
Please find enclosed the preliminary jurisdictional determination (PJD) request, Low Impact Bridge
Replacement (LIBR) package and permit drawings for the above referenced project that was prepared for
NCDOT by HTNB and ESI.
Project Description:
The purpose of this project is to replace Sampson County Bridge No. 148 on SR 1434 over Mill Creek which
is classified as C;Sw waters in HLJC 03030006. Bridge No. 148 is currently 35 feet long with 25.3 foot deck
width. The replacement structure will be an 85 feet long single span bridge with a 33 foot total deck width.
The bridge will include two-ll-foot lanes and 4.0-foot offsets. The bridge length is set by hydraulic
requirements.
The approach roadway will extend approximately 238 feet from the southeast end of the new structure and
137 feet from the northwest end of the new bridge. The typical cross section of the approaches will include 2
— 11" asphalt paved lanes with 2 ft. paved shoulders and 6 ft. grass shoulders. A soil driveway will have to
relocated from Sta. —L- 19+71.75 to 19+25 RT. The roadway grade of the new structure will be
approximately the same as the existing grade.
The proposed bridge does not contain deck drains and all stormwater from the bridge and approaches will be
collected by a stormwater drainage system and discharged into a Class B rip rap pad at —L- 19+36 RT.
Jurisdictional Imp acts:
There is a mill pond upstream of the bridge and the discharge from the dam has create substantial scour on
the downstream right bank therefore, 61 linear feet of Class II rip rap bank stabilization will be required on
from Sta. —L- 18+92 to 19+01. Since the proposed slope is less than 1.5:1, NCDOT would like to a waiver
from the filter fabric requirement so the rip rap can be pressed into the bank for better placement and
stabilization.
Minimal wetland impacts (0.01 acres of inechanized clearing and fill) are proposed in the southeast quadrant
along the shoulder.

Purpose and Need:
The existing structure is structurally obsolete and structurally deficient with a sufficiency rating of 28.36 out
of a possible 100 for a new structure.
Utilities:
Based on the Preliminary Utility Analysis and Progress Report, the only utility conflict on this project is the
SREMC powerline which will have to be shifted to the NNE. There are no jurisdictional impact associated
with this relocation.
Regulatory Approvals:
Section 404 Permit: We anticipate USACE Nationwide 3 and 13 will apply to this project. Written
concurrence from the USACE is not required for this project. NCDOT will abide by all conditions listed in
Nationwide 3 and include a copy of the certification in the permit package and/or contract.
Section 401 Permit: We anticipate 401 General Certification 3883 and 3885 will apply to this project.
Written concurrence from the Division of Water Resources (DWR) is not required for this project. NCDOT
will abide by all conditions listed in WQC 3883 and include a copy of the certification in the permit package
and/or contract.

Project Description: The replacement of Bridge No. 148 oer Mil1 Swamp on SR 1434 (Huntley School
Road) in 5ampson County, North Caroiina. The archaeologial Area of Potentiai Effects {APE) is
centered on the hridge structu'e and measures 66ft. in length (3Uft, from each bridge end-point} and
150ft. in width {75ft. fram each side of the SR 1434 center-line).
SUMMARY QF ARCHAEULUGICAL FINDIIGS
The Nortlr Carolina Deparment oj Tratsporlrrtion {NCDDTj Arcltnevlogy Graup reviewed Nte subject
project and determed.•
There are no National Register listed or eligible ARCHAEOLGICAL SITES present
within the project's area of potential effects. (Attah any notes or documents as needed)
No subsurface archaeolagical inestigativns were required for this project.
❑ Subsurface inestigations did nat reeal the presence f any arhaeolagical resources.
❑ Subsurface inestigativns did not reeal the presence af any archaeolagical resources
considered eligible for the National Register.
❑ All identified archaealogical sites located within the APE hae been considered and all
compliance for archaealogical resources with Sectin ld6 of the National Historic
Preseration Acf and GS 121-12(a} has been completed for this prvject.
Brref rlescriptian of review artivitres, resulls nJreview, arr! conclusiars:
Ta determine the cultural resaurce ptential of the APE, numerou5 sources af information were
cansidered. First, preliminary constructinn design, funding, and other data was examined for defining
tFe patential impacts to the APE grvund surfaces and far determining the level of effort necessary for
campliance. In this case, the project is state-funded with federal permit interaction and subject to
5ection 1D6 of the National Historic Preservation Act. Next, a map review and site file search was
canducted at the Dffice of State Arehaeology {OSA) on Thursday ctober 6, 201fi. Dne preinusly
documented archaeologica! site 31SP235) has been recorded directly adjacent to the project's APE.
Identified as Matthew'S Miil on the archaeological site farm, the site was recorded by the SA in 1981.
31SP235 constitutes the aboe-ground remains of a dam, mill, and mill pond. This cultural resaure
remains unassessed for Natianal Register of Histnric Places NRHP} eligibility.

Examination of NRHP, 5tate 5tudy Listed {5Ly, Lacalfy Oesignated {LDy, Determined Eligihle (Ej, and
5urveyed 5ite (SSj properties employing resources available on the North Carolina 5tate Historic
Preservation Dffice NC5HP0) website illustrated that the Matthew's Mill location is also known as the
Rob Crumpler Mill and situated in the southeastern project quadrant but beyond the limits af the APE. It
is currently unknown if any other abave-graund or subsurface remnants af the mill may extend into the
currently defined APE. Furthermore, historic maps af Sampsan County were appraised for former
structure locatians, land use patterns, or other canfirmatian af histari occupation in the project vicinity
and archaealogical/hi5torical referenee materials were reiewed as well.
In addition, topvgraphic, eningic, flaad boundary, lidar, and NRCS soil survey maps were referenced for
the ealuation of geomorphalagical, pedeological, hydrological, and other environmental-type elements
that may hae resulted in past nccupation at Chis location. Finally, review af aerial and on-grnund images
NCDOT Spatial Data Viewer, Goagle, ARC-G15j afforded first-hand per5peetives of the overall study area
which were useful for assessing localixed disturbances, bath natural and human induced which
cnmpromise the integrity of archaeofogical site5/deposits,
Based on the presene of a previously recarded archaeolagical site 315P235j an in-field reeannaissance
and survey of the site [acation is reeommended prior to con5truction/replacement activities. This survey
work will seek to determine if any portion of 315P235 extends inta the APE and will ealuate the
resaurce for NRHP eligibility if contained within the project boundaries.
A reconnaissance inestigation of the project area was completed hy NCOQT archaeologists 5cott
Halnrsen and Damon Jones on Friday, Oetober 28, 2016. No portian of 31SP235 is located within the
pre5ently defined APE. The mill structure is located a caupie of hundred feet fram the eastern APE
baundary. Also, there are no mill efements {mill race, dam, etc.} cantained in the APE. The mill dam,
situated east of the APE, was breeched during Hurricane Mathew by the falling of a giant tree. The
entire contents of the mill pond emptied over and through the dam flooding the entire APE. During aur
isit, some two weeks later, eidence of the flood was still isible in the APE. 7he northern and
southwestern quadrants still contained ponded water, while the southeastern quadrant is a natural spill-
way fnr the dam. Because of these condtions, and since the APE is already underlain hy frequently
flooded and poorly drained soils (BHj, no shovel testing was necessary ta confirm the absence af
significant archaeological re5nurces. Na further arehaeological wor[c or consultatian will be needed for
the project as proposed.

Swamp (off-site detour planned, assume no improvements).
SUMMARY OF HISTORIC ARCHITECTURE AND LANDSCAPES REVIEW
❑ There are no National Register-listed or Study Listed properties within the project's area of
potential effects.
❑ There are no properties less than fifty years old which are considered to meet Criteria
Consideration G within the project's area of potential effects.
❑ There are no properties within the project's area of potential effects.
❑ There are properties over fifty years old within the area of potential effects, but they do not
meet the criteria for listing on the National Register.
X There are no historic properties present or affected by this project. (Attach any notes or
documents as needed.)
Descrition ofreview activities, results, and conclusions: HPOWeb reviewed on 1 November 2016
and yielded two SS and no NR, SL, DE, or LD properties in the Area of Potential Effects (APE). The APE
for historic architectural resources extends 300 feet from either end of the existing bridge (NW-SE) and
150 feet to either side of the SR 1434 (Huntley School Road) centerline (NE-SW) to encompass all
anticipated construction activities and likely impacts. Sampson County current GIS mapping, aerial
photography, and tax information reveal a mostly wooded APE with several residential and agricultural
resources dating mainly from the 1920s to the 2000s. The circa-1922 Robert Crumpler House (SP0511)
sits approximately 275 feet NE of the SR 1434 (Huntley School Road) centerline, beyond likely project
impact; no other buildings or significant landscape elements appear on the property, which, however,
may be related to mill resources located further to the SE (#1324 Huntley School Road, PIN:
06064162001). The APE intersects a parcel on which an unexceptional, circa-1940 house is located
about 480 feet NW of the existing bridge (#1407 Huntley School Road, PIN: 06065220006). SE of the
bridge, the APE intersects parcels (PINs: 06064220009 and 0605582003) containing two unexceptional
houses and various outbuildings dating to the first half of the twentieth century, a cemetery located
about 350 feet NE of the SR 1434 (Huntley School Road) centerline, and the above-ground remains of a
mill, dam, mill race or sluiceway, and mill pond, all but part of the race/sluiceway outside the APE
approximately 290 feet and more E of the bridge. The mill resources are recorded in the state
architectural survey as the Rob Crumpler Mill (SP0512); they appear in the state archaeological record as
Matthew's Mill (31SP235). Constructed in 1954, Bridge No. 148 is not eligible for the National Register _

according to the NCDOT Historic Bridge Survey as it is not representative of any significant engineering or
aesthetic type. There are no National Register-listed properties in the APE or along the proposed off-site
detour route. Initial screening of the project suggested that additional investigation of the mill resources
might be needed (survey required form, 7 November 2016), but additional design and permit information
received from Division 3 indicates that a finding of "no historic properties affected" will satisfy both
Section 106 and GS 121-12(a) compliance requirements.
Should any aspect of the project design change, please notify
NCDOT Historic Architecture as additional review may be necessary.
