Duplin County NC WBS#: DF15003.2031012 (DWQ Permit Fee: $240) Dear Mr. Shaver and Ms. Steenhuis, The North Carolina Department of Transportation is planning to replace the existing one 48" CMP x�70 LF at location Lat: 35.071740 Long: -77.854294 on SR 1566 Wallace Walker Road 0.5 miles West of SR 1501 (Woodland Church Rd) with (1) @ 60" CMP x 42 LF w/Headwall (See Attached Documents). NCDOT would like to request a waiver from the burial requirements in Regional Condition 3.6.1 of the Nationwide Permit 3 and General Condition 11 of the corresponding Water Quality General Certification No. 4085. The 60" CMP will be placed on a 2% slope to provide a low flow condition. Due to the scour and the perched outlet of the existing CMP, 14 LF of rip rap will be placed on outlet end in the existing footprint of the existing 48" CMP on a 2% slope to match pipe slope and tie into existing streambed elevation to protect stream channel from scour and destabilization. (See Attached Permit Drawing). By copy of this letter and enclosures, NCDOT is requesting issuance of Nationwide 3 approval from the US Army Corps of Engineers and 401 Water Quality Certification (WQC) #4085 approval from the Division of Water Resources (DWR). A method of debiting $240 to process the WQC request has been provided as noted in the subject line of this application.
