The following personnel was in attendance for the review: Jeff Winkler, Greg Johnston, Tim Latham (Land Quality), Dave Wanucha (DWQ), and Charlie Brown (Vecillio). The contractor has completed several of the maintenance and repair items that were listed in the previous inspection report. However, the project has continued to receive additional rain which has delayed access to some of the slopes and waste sites that need attention. Therefore, the ICA will be extended for another 5 working days or until all the work has been completed. Below are recommendations for areas where some repairs have been made -Sta. 346 RT - Contractor has installed a temporary pipe to convey the stream into an existing 36 inch pipe. Remove the dirt and debris from the inlet of the existing pipe and line inlet with plastic and rip rap. Also, install silt fence along the R/W boundary, and around the inlets of the temporary and existing pipes. Dress and stabilize the remaining disturbed areas. -Sta. 348 LT - Inlet protection needs to be installed around the slope drain ASAP. A TSD has been installed to divert runoff into the slope drain. Make sure the berm of TSD is built as per spec. C� 0 11/09/2017 2:39 PM Page 1 of 2 -Miller Waste Site- Hand remove any sediment in the ESA. Repair silt fence and SSCF outlets protecting the ESA. The basin has been temporarily repaired. The weir needs to be installed as per spec. Ensure the weir is level and geofabric has good contact with the soil. Dress and stabilize any disturbed areas adjacent to the basin. Remove any accumulated sediment from basin, and install TSDs as per plan. -Sta. 333 RT - The water level has receded since the previous inspection. Use basin 10.2 as a stilling basin. This basin needs to be repaired prior to using. Once the area is dry, then remove sediment from the ESA, and reinstall EC measures as per plan. **Continue to install and maintain EC devices that were listed in the previous inspection report. Make sure that skimmer basins are constructed according to specifications. Continue to dress and stabilize areas after repairs are made. 11/09/2017 2:39 PM Page 2 of 2
