NCDOT Inspector, Gregg Hamm, and State Roadside Environmental Engineer, Ben DeWit, was present during the project inspection. The project received 5+ inches of rain on October 23. As a result, many EC devices throughout the project failed and there were substantial amounts of off-site sediment in various locations. A project inspection was performed on 10/24. Many areas of the project were inaccessible due to the wet conditions. The project was suspended by the Resident Engineer on 10/24 and all crews were tasked with focusing on erosion control. A follow-up inspection was scheduled for 11/1. The suspension was lifted by the Resident Engineer on 11/2, and the follow-up EC inspection occurred later that day. Many areas were identified that are still out of compliance. 7 0 11/03/2017 1:59 PM Page 1 of 3 PROJECT RECOMMENDATIONS Poling Waste Site -Site is out of compliance. Send the reclamation plan back to the consultant to design an additional basin for the southeast corner of the site. This drainage area was designed to drain to basin SB-3; however, this does not seem feasible due to the clearing limits and existing topography. The contractor has installed a small silt basin; however, the basin was not designed for the drainage area, and was not constructed per NCDOT specifications. Poling Waste Site - Basin SB3- Contractor was on site repairing this basin during the inspection. Make sure that the basin is installed per NCDOT specifications. Geo-fabric needs to cover the entire back slope of the weir section to prevent slope from eroding during rain events. Poling Waste Site- Basin SB2 - Basin needs to be constructed per NCDOT specifications. Install baffles and seed and mat the interior and exterior side slopes. Cover weir and back slope with geofabric. Silt fence adjacent to the basin is not keyed in per spec and is not effective. Poling Waste Site- Northern side of the site has been cleared and grubbed. EC measures are not in place. Install basin SB1 ASAP. Make sure that runoff is diverted into the basin per plan. Poling Waste Site- TSDs need to be installed per plan throughout the site. All TSDs need to divert runoff into a basin per plan. Install an adequate number of TRSC-Bs in TSDs to control the velocity of runoff. Sta. 299 RT - Skimmer basin 7.1 has recently been removed. Inspector stated that the site would not be graded until this spring. A skimmer basin needs to remain in place if the site is to stay in this condition through the winter. There is steep terrain and more than 1 acre of disturbance that is draining to this location. This site is an environmentally sensitive area. Sta. 302 LT - Skimmer basin 7.3 has not been installed per plan. This resulted in a sediment loss at this site. Construct basin ASAP. Sta. 302 RT- EC measures have not been installed around the inlet of the new pipe per plan. Seed and mat any disturbed areas around the silt fence ASAP. Divert runoff into the tiered skimmer basin ID 7.4. TDs can be cut along the slope into the first cell of each basin. Reinstall the third tier of the basin. Miller Waste Site- Contractor is dewatering the skimmer basin into a special stilling basin in order to finish construction of the weir. The special stilling basin has not been installed per specifications and in not effective. High turbidity water was being discharged into the stream. Ensure that the rock pad is level and at least 8 inches thick. Discharge from the silt bag should be kept clean before entering the stream. The pump was shut off during the project inspection. In addition, skimmer basin needs geofabric and skimmer reinstalled. Sta. 346 RT - A live stream is currently flowing over disturbed ground for 500 ft. before being discharged into the JS at Sta. 351 RT. The pipe that the stream originally flowed through is completely blocked by sediment. This pipe needs to be restored or the stream needs to be diverted through another pipe or a lined temporary channel. Sta. 351 RT - Silt fence and TRSC-As need to be reestablished ASAP. Sta. 348 LT - Silt fence has been repaired. However, a SSCF outlet is still overtopped with sediment, and large washes in the slope have not been repaired. NCDOT Waste Pit - Skimmer basin has not been constructed per spec. Weir needs to be reconstructed. Ensure outlet pipe is placed 1 ft. above the bottom of the basin. Geo-fabric needs to be reinstalled on weir section. Baffles need to be installed. Seed and mat interior slopes of the basin. Sta. 368 RT - Skimmer basin 12.1 needs to be reestablished by installing baffles, reconstructing weirs, replacing fabric, and dressing the outlet. Sta. 374+56 RT - Temporary pipes to convey stream have been installed for haul road. Inlet of pipes have no EC protection in place. Install silt fence around inlets and direct runoff through a TRSC-A in the ditchline. Sta. 394 to 396 RT - Install silt fence around the JS per plan. Sta. 411 LT - Install skimmer basin 16.1 per plan. Utilize the existing topography for the side slopes. Install weir with skimmer pipe and baffles per spec. 11/03/2017 1:59 PM Page 2 of 3 Blackburn Waste Site- Reestablish the ditchline and EC measures along the back slope of the site. Consider placing riprap in the ditchline to prevent future erosion. Remove any off site sediment where the silt fence had failed. Keep RISTs on slope drain pipes. Sta. 317 LT - Silt fence needs to be reinstalled to protect the outlet of the pipe. Maintain basins 9.1 and 9.2. Ensure that runoff from the access road is diverted into basin 9.1. Install TRSC-A for outlets per plans. Sta. 263 to 268 +/- LT - Reestablish ditchlines. Ensure that wattles make good contact with the ground. Water is currently running under many of the wattles. **EC maintenance including replacing wattles, repairing washes, refreshing sediment control stone and removing silt needs to be done throughout the project. NPDES records were up to date; however, records did not reflect the amount of work that needs to be completed for the project to be in compliance. Ensure that SEDIMENT DEPOSITED is marked YES if sediment has left the site. -Project will be reviewed again for compliance in 5 working days. 11/03/2017 1:59 PM Page 3 of 3
