Responsible party: PSNC Representative—Reid Miller Telephone (919) 819-2068 Contractor performing HDD: ITS Directional Drilling Date and time of IR: Saturday, October 21, Monday October 23, and Tuesday, October 24, 2017 How was IR detected: Reid Miller (PSNC) noticed a small IR on east bank of the unnamed tributary. Location of the IR: UnnamedTributaryflowing into Falls Lake at Camp New Life approximately 145' east of the Koupela Drive/ Durant Road Intersection. See attached map. Type of material: Bentonite clayand waterslurry — standard HDD drilling mud. Description of IR and effortsto protect sensitive areas: The drilling operation was stopped immediately upon noticing the drilling mud in the unnamedtributary.The source of the drilling mud was identified to be atthe toe of the slope on the east bank. Sand bags were installed to contain the drilling mud and a vacuum truckwas obtained. The vacuum truck removed the drilling mud from the containment area and reclaimedthe drilling mud. No additional dischargeswere identified after drilling operation had been stopped. No drilling was conducted on Sunday, October 22, 2017. On Monday, October 23, 2017 the contractor resumed drilling operations after no additional inadvertent returnswere observed since the initial return observed on Saturday, October2l, 2017. The site of the original IR began to discharge again and drilling was immediately stopped. Sand bagswere still in place to contain any potential dischargesfrom the IR and a vacuum truckwas utilized to remove and reclaim the drilling mud that had been released into the unnamed tributary. Drilling was stopped while the IR was being assessed and the impactswere being mitigated. Drilling resumed on Wednesday, October24when no additional dischargeswere identified sincework had stopped. When the IR began to discharge again, drilling operationswere immediately ceased. The sand bags continued to contain the discharge and thevacuum truckwas again utilized to remove the drilling mud. Ashley Bentz reported incident to Jennifer Burdette of the North Carolina Department of Water Resources (Raleigh Region) and Tasha Alexanderofthe U.S. ArmyCorps of Engineers (USACE) byemail on Friday, October27, 2017. Estimated size of IR (quantityand area): A fine layer of drilling mud impacted the tributary bed for approximately20 feet. Due to the quick action of the contractor and environmental inspector, a minimal amount of drilling mud entered the stream priorto stopping the drilling operation and establishing containment. Estimated impact was approximately 50-100 gallons. Proposed additional HDD activities: The designed length of the HDD is approximately 1,000 feet. The pilot hole should be completed by Friday, October 27, 2017. There will be one ream (12" reamer) afterthe pilot hole is completed before the pipe pull back and it will be less than one week before the entire HDD is complete. The HDD will be monitored for any additional IRs and if they occur, ITS will continue to contain the discharge and remove any discharge via a vacuum truck. It is possible that pressure will be relieved upon daylighting the pilot hole, thereby reducing the potential for future IRs. Proposed final cleanup method: On completion of HDD activities, ITS will remove any drilling mud and turbid waterwithin the containmentarea. The mud may be removed both by hand and with a vacuum truck. Upon completion ofclean-up activities, sandbags and other containment materialswill also be removed. Party responsiblefor cleanup: ITS Directional Drilling General comments: It appears that the drilling mud has been contained within the sandbagged area, which is supplemented bya natural dam within the stream bed. The contractorstopped drilling operation, acted in good faith to contain the drilling mud and contacted environmental monitors. The contractor and inspectors should remain alert for rain events and should take actions necessaryto contain drilling mud and turbid water in the impacted area. Effortsto clean up the impacted area should start as soon as practicable following bore completion and on the occasion of an additional IR. Attachment: IR Report Site Photographs 1-4. Sand bags have been installed upstream of the IR to prevent clean water from entering the containment area. There is a natural berm within the stream which is also serving to contain the drilling mud and turbid water.
