REQUEST FOR MORE INFORMATION
Dear Ms. Gooch:
On December 1, 2008, the Division of Water Quality (DWQ) received your application dated
November 22, 2008, to impact 0.0053 acres of wetlands to construct a driveway to access a
telecommunication tower. The DWQ has determined that your application was incomplete
and/or provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected wetland, streams and/or
buffers on the subject property. Therefore, unless we receive the additional information
requested below, we will have to move toward denial of your application as required by 15A
NCAC 2H .0506 and 2B .0233(8) and will place this project on hold as incomplete until we
receive this additional information. Please provide the following information so that we may
continue to review your project.
Additional Information Requested:
1. Nationwide/General Certification Request
Within the PCN application you are requesting to utilize a Nationwide 12 permit. The
DWQ General Certification 3699 coincides with the NW 12 permit for utilization with
utility impacts to wetlands. Section III, 11, of the PCN form indicates that the purpose of
the impact is to provide an "access road". The DWQ 3699 does not cover the installation of
an access road. An access road would be covered under the GC 3704 (NW 14). Please
contact the US Army Corp of Engineers to determine if you have the appropriate permit
request on the PCN application. If your request is changed to a Nationwide 14 by the

USCOE you will need to provide the DWQ a modified page 5 of 13 and the Minor
application fee of $240.
Please respond in writing to the DWQ within four weeks of the date of this letter to with the
requested information. If we do not hear from you within four weeks, we will assume that you no
longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Kyle Barnes of the Washington
