Please find the attached Pre-Construction Notification (PCN) Form, including supplemental information
and Water Quality Certification Fee for Minor Water Quality Applications ($240). ECS respecHully
requests the use of WQC #4088 for a roadway extension. The proposed impacts to waters of the US
(wetland WA and stream SA per the attached impact maps) are for the purpose of the construction of a
road extension to connect the main campus of GTCC to the south west corner of the property located at
6012 W. Gate City Blvd. in Greensboro, NC. A 50 foot right of way will be used for roadway construction
of a two lane paved road and installation of a 60 inch diameter RCP pipe. The proposed permanent
wetland impacts will consist of inechanized land clearing, excavation and fill. Temporary wetland impacts
for hand clearing in wetlands are also proposed. There are a total of 0.117 acres of permanent wetland
impacts and 0.037 acres of temporary wetland impacts proposed. The proposed permanent stream
impacts will include culverting and rip rap fill. Temporary stream impacts for dewatering are also
proposed. There are a total of 107 linear feet of permanent stream impacts and 13 linear feet of
temporary impacts proposed. Riparian buffer impacts are being coordinated with the Town of Jamestown.

The project area is located east of the GTCC campus. The project area currently contains a natural gas and power line
easement. The area is undeveloped with the exception of natural gas monitoring station and power line towers. The
project area will border the ROW for Guilford College Road.

To preserve the safety and mobility of the travelling public through the connection of parking lot at 6012 W. Gate City Blvd
with the main campus of GTCC.

GTCC to install one 60" @ 64' Class III CMP buried at a minimum of 1' with an aluminum inlet and outlet headwalls
(8.125' with one 2.125' wing and one 6.5' (16.75' total)). Class 1 Rip Rap would be placed upstream and downstream of
the structure inlet and outlet for flow redirection and structure protection. Standard road construction and pipe
replacement equipment such as cranes, excavators, dump trucks, and similar vehicles would be used for the project.
Proper erosion and sedimentation control measures would be employed throughout the project.

1. Avoidance and Minimization
1 a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
The tie-in to Stanford Road was fixed in location because of existing private property and the Fire Training Center. There is
limited GTCC road frontage on opposite side of the road restricting potential crossings. In addition, the utility crossings of an
existing gas distribution line and Duke Power transmission towers had to be managed. The Railroad crossing in close
proximity impacted availability of crossing options as well. The road grade was dictated by the tunnel elevation. In order to
minimize the impacts to the stream and wetlands, a narrow roadway and 2:1 side slopes were utilized. The design follows the
existing topographic contours and the rip rap fill is a minimum to protect the structure and reduce flow energy. A new stream
alignment is proposed to restore a more natural pattern and profile to the system. To avoid widening the stream channel or
reducing the depth of the stream in connection with this construction activity the pipe will be aligned with the stream channel
and will be buried approximately 1 foot below the natural stream bed. NCDOT design recommendation is minimum
requirements for State standards.

The purpose of this letter is to notify you that the NCDEQ Division of Mftigadon Services (DMSj is willing to
accept payment for compensatory mftigation for impacts assodated with the above referenced project as
indicated in the fa61e below. Please note that this decision does not assure that participation in the DMS in-
lieu fee mitiga[ion program will be approved by the permit issuing agencies as mRigadon for project impacts.
It is the rasponsibility of the applicant to contact pertnitting age�cies to determine if payment to the DMS wiil
be approved. You must also comply with all other state, federal or local gavernment pertnits, regulations or
authorizations associafed with the proposed adlvlty including G.S. § 143-214.11.
This acceptance is valid for six months from the date of this letter and is not transferable. If we have not
received a copy of the issued 404 Permitl401 CertificationlCAMA permlt wlthin this time freme, this
acceptance will expire. It is the applicanYs responsibiliry to send copies of the permits to DMS. Once DMS
receives a copy of the pertnft(s) an invoice will be issued based on the required mitigation in that permit and
payment must be made prior to wnducting the authorized work. The amount of the in-lieu fee to be paid by
