I don’t think it is too big. Find attached PDF of lining calculation, StormEZ cover and bioretention supplement.
Email for Mr. Stephenson: jeff.stephenson.cf5k@statefarm.com
I believe these items addess your request. Please let me know if you have any questions or need further information

Blair – I’ve sent in a help ticket (I thought they corrected that on the form now). Depending on the size, you can try
emailing it to me until they fix the form. Thanks.

Mr Patterson, the DWR# referenced in your email SW6170901, is showing as invalid on the website upload form. Is this
project still in the Pre‐Application stage, or is it considered an existing project and I have an incorrect SW number?
thanks

Subject: SW6170901 ‐ Oak Hill Living Center ‐ Request for Additional Information
Blair – This is a follow‐up to the voice mail I left you this morning.
Please provide the following additional information to me by November 16, 2017.
1. Provide an email address for the owner, Jeffrey Stephenson.
2. Provide supporting calculations to demonstrate the swales will be non‐erosive for the 10‐yr storm
event. Information about swale vegetation type and liners was not provided.
3
3. Provide a completed bioretention supplement using the Supplement‐EZ form:
https://deq.nc.gov/about/divisions/energy‐mineral‐land‐resources/energy‐mineral‐land‐permit‐
guidance/stormwater‐bmp‐manual
4. Implementing a bioretention cell for this project as proposed is allowable. However, I must mention that the
calculation for low vs. high density is typically based on the property area (not sub‐drainage area). Based on the
info provided in the application, and 15A NCAC 02H .1003(1), it appears this project may be designed as low
density. If you decide to pursue a low density permit, please use our regular application form (SWU‐101), low
density supplement, and deed restriction form on our website: https://deq.nc.gov/about/divisions/energy‐
mineral‐land‐resources/energy‐mineral‐land‐rules/stormwater‐program/post‐construction The Storm‐EZ form
submitted has not yet been updated to incorporate low density or the new MDC adopted this past January.
Please let me know if you have any questions in this matter. Thanks.
