REQUEST FOR MORE INFORMATION
Dear Mr. Thompson:
On September 3, 2008, the Division of Water Quality (DWQ) received your application dated August 30,
2008, with additional information received from you on September 9, 2008, and December 1, 2008, to fill
or otherwise impact 78 linear feet perennial stream impact, 70 linear feet intermittent stream impact, and
7,200 square feet of Zone 1 Neuse River basin protected riparian buffers and 4,000 square feet of Zone 2
Neuse River basin protected riparian buffers to construct the proposed residential development at the site.
On September 9, 2008, the DWQ received additional information from you, however, the DWQ has
determined that your application remains incomplete and/or provided inaccurate information as discussed
below. The DWQ will require additional information in order to process your application to impact
protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the
additional information requested below, we will place this project on hold as incomplete until we receive
this additional information. If we do not receive the requested information, your project will be formally
returned as incomplete. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
1. Please provide a copy of the overall site plan that depicts the location of Phase 2 with regard to
Phase 1.
2. Please provide diffuse flow measures for both the east and west sides of the stream crossing at Sta
15+50 and the Northeast quadrant of the stream crossing at Sta 00+75 of Chestnut Ridge Road.
This includes complete level spreader supplement forms and all required items, available at:
http://h2o.enr.state.nc.us/su/binp_forms.htm.
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter, your project will be withdrawn
and you will need to reapply with a new application and a new fee.
