1. Please provide a copy of the overall site plan that depicts the location of Phase 2
with regard to Phase 1.
2. Please provide diffuse flow measures for both the east and west sides of the
stream crossing at Sta 15+50 and the Northeast quadrant of the stream crossing at
Sta 00+75 of Chestnut Ridge Road. This includes complete level spreader
supplement forms and all required items, available at:
