Please see the responses below regarding the Slab Town Road project:
1. Please provide a typical or construction sequence showing how the proposed work will meet Condition 9 of
Water Quality Certification 4092, which states that "all work in or adjacent to streams shall be conducted so that
the flowing stream does not come in contact with the disturbed area." [15A NCAC 02H .0506(b)(3)]
A pump around system/apparatus would be utilized to divert flow within S1, S2, and S4 during utility line
installation. Stream flow would be diverted downstream, beyond the utility line corridors/limits of
disturbance. This system would incorporate the use of a temporary, impervious dike near the water intake
area. Water would then be collected via an intake hose and pumped around the existing stream channel via a
discharge hose. The discharge hose would be attached to a dewatering bag for sediment filtration, and would
be located downstream near the discharge point on the existing stream channel. The impervious dike used to
capture water near the intake hose would be removed upon the completion of construction activities.
2. Figure 3.2 depicts the waterline easement and sewer line easement running parallel to each other at impact
W1. Can the easements be combined to minimize impacts to Wetland 1? Please re -configure the lines to utilize
the same easement or clearly explain why the impacts cannot be minimized. [15A NCAC 02H .0506(f) and (g)]
A portion of the property parcel on which the proposed project is located was been placed into a conservation
easement that was recorded in March 2008 following the initial permit process. The utility easements
referenced in this application were defined during the initial permit process and have already been surveyed
and incorporated within the designated easement area.

On September 11, 2017, the Division of Water Resources (Division) received your application dated September 7, 2017,
requesting a 401 Water Quality Certification from the Division for the subject project. The Division has determined that
your application is incomplete and cannot be processed. The application is on hold until all of the following items are
addressed: [15A NCAC 02H .0507 (c)]

Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the
proper consideration of the application. If all of the requested information is not received by October 19, 2017, the
Division will be unable to approve the application and it will be returned. The return of this project will necessitate
reapplication to the Division for approval, including a complete application package and the appropriate fee.
