Dear Mr Swain
On December 12, 2008, I visually inspected the construction work on the US Highway 19 project in Madison
and Yancey Counties Work is well underway in both Madison and Yancey Counties with heavy construction
activities taking place on both projects
The Division of Water Quality has concerns over the construction because some sediment and erosion control
devices failed during recent rain events and sediment entered State surface waters We cannot emphasize
enough the need for proper installation and maintenance of sediment and erosion structures including
straw/mulch, sediment basins, sediment control fencing, check dams, polyacrylamide, waddles, floc logs, etc
Please continue to take necessary steps to prevent sediment from entering streams and protecting State surface
waters Please refer to the attached inspection report for specific items noted during the inspection

Additional comments 7,059 linear ft of stream impacts and 0 42 acres of wetlands for R-2518A,
3,675 linear ft of stream impacts and 0 23 acres of wetlands for R-2518B Compensatory
mitigation for impacts of 5, 453 linear feet is required
1 Grading and construction work is well underway from one end of the project to the other
2 One box culvert has been completed and two others are presently under construction
3 Grading is taking place along the entire length of the project with heavy grading taking place on the
mountain part in Madison County and between Bald Creek and the Cane River in Yancey County
4 Some erosion control devices were full of silt and had overflowed into the stream These should be
repaired and additional controls should be constructed as necessary
5 Repair gullies in haul road going to the fill area above Holland Creek Please ensure fill area are
mulched and seeded properly and in a timely manner to control runoff
6 Additional sediment and erosion control measures are needed on the cut slopes adjacent to US
Highway 19 near Stations 88+00 to prevent sediment from entering State surface waters
7 It appears that the amount of straw mulch and tack being applied is somewhat sparse It is
recommended that these items be increased in future seeding operations
8 Any visible sediment in streams should be removed by using hand tools If there are questions
regarding sediment removal please contact Mike Parker at 828-296-4500
9 As a reminder the streams on the Madison County construction area are classified as High Quality
Waters and all measures necessary to protect State surface waters should be employed
