JAY ZIMMERMAN Director On September 20, 2017, Roberto Scheller of this Office conducted an inspection at the subject facility This inspection was conducted as a Compliance Evaluation Inspection (CEI) to insure compliance with permit requirements and conditions It was noted during the subject inspection and during previous inspection conducted on April 11, 2016, by Mr On Tuvia, that the effluent pipe going to holding tank was leaking. Please be advised that in accordance with your current NPDES Permit Number NC0075523, Part Il, Section C, Item 2 "Operation and Maintenance", and Item 4.
