The Division of Energy, Mineral, and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on June 27, 2017. A preliminary review
of that information has determined that the application is not complete. The following
information is needed to continue the stormwater review:
1. On SWU-101 items IV. 8 and 10, complete all impervious area information in square
feet.
2. The following forms must be completed and originals submitted:
a. Supplement -EZ form
b. O&M Agreement EZ form
c. High Density Residential Deed Restrictions form

3. Since the project proposes handling runoff predominantly using storm drainage piping,
SCM's, and is not dispersing flow, it must meet all of the high density requirements and
MDC's in the SCM Design Manual and the revise 2017 rules. Revise the SCM to be
from the list ofrip marX SCMs to treat the water quality volume (1"). Dry ponds are
secondary SCM's.

4. Provide supporting calculations for the SCM water quality designs to demonstrate that all
MDC are met (ex. WQV, drawdown time of WQV, SA size, anti -floatation, etc.). See
the General MDC and the SCM specific MDC.
5. Show on the plans the required access and maintenance easement around each SCM to
the public r/w.
6. Add the impervious area in square feet to the SCM DA map (sht. APP -B).
7. Due to the nature of the above comments, additional comments may be forthcoming once
revisions are submitted.
8. Please provide revisions both in hard copy (2 sets of revised plan sheets) and pdf (email,
ftp, cd, or flash drive).
Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to August 14, 2017, or
the application will be returned as incomplete. The return of a project will necessitate
resubmittal of all required items, including the application fee.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
