Remarks and Recommendations:
Project is out of compliance due to failure to provide adequate installation of bypass pipe with substantial sand
bags for impervious dikes.
Project was re-reviewed by Roadside Environmental Field Operations Technician Kim Whitley.

The project received rainfall the weekend of September 2nd causing the impervious dikes and diversion pipe to
fail. The project was issued a Temporary Suspension of Work by the Resident Engineers Office on September
5th. The project was reviewed on September 6th and an EC deficiencies list was sent to the contractor that
same day. The project was re-inspected today, September 13th. At the time of review, impervious dikes and
diversion pipe had not yet been re-installed to provide containment of the work area. See corrective actions
needed below:
Note corrective actions:
- Install new 48 inch bypass pipe with substantial sand bags for impervious dikes.
- Install diversion across the grade at approximate Sta 16+/- to divert runoff to the Rt side of the project. Slope
drain should be installed at the end of the diversion to convey runoff down the slope to a dissipator pad. SSCF

outlet should be installed in the SF at the outlet of the slope drain.
- Clean up oil spill in the center of the roadway immediately down slope of the conex.
- Clean up oil spill on the shoulder vicinity Sta 17+35 Rt. Dispose of contaminated soil at an approved site.
- Sta 15+40+/- Rt. - Filter stone at top of slope needs pulled back, so it does not enter the Jurisdictional waters.
- Sta 15+85+/- - Improve ground coverage on slope prior to culvert.
- Sta 15+75+/- - Bare area below coir fiber mat needs stabilized with seed/coir fiber mat.
- Ensure to apply additional ground coverage to other areas damaged by foot traffic throughout.
NPDES paperwork was reviewed. Corrections have been made. Continue with NPDES inspections and
document once every 7 days or within 24 hours of 0.5 inches of rainfall in a 24 hour period, per the NCG01
Permit Requirements. Mark and date as built e. c. plans, as to what measures are on the ground.
The project will be re-inspected within 5 working days.
