The Request for More Information Letter sent on October 24, 2008 stated,
"Please submit a Level Spreader Supplement Form with all required items
for each proposed level spreader (see
littp://h2o.enr.state.nc.us/su/bmp forms.htm)." Your most recent
submittal for this project failed to provide the one-foot topo lines between
the level lip and top of the stream bank as well as a date-stamped
photograph of the filter strip that clearly shows the type of vegetation that
is present. The DWQ cannot review the diffuse flow design without this
information.
