On May 23, 2016 and March 22, 2017, the Division of Water Resources (DWR) received close-out
packets from you on behalf of the Division of Mitigation Services (DMS) requesting approval to close-
out the subject site from monitoring and maintenance of the riparian areas. On June 21, 2016, Katie
Merritt with DWR, visited the subject site to assess the vegetation and overall site conditions. On June
22, 2016, Ms. Merritt provided correspondence to DMS documenting issues with closing out the site. On
May 10, 2017, Ms. Merritt and Sue Homewood with DWR, revisited the site and determined that all
issues raised by Ms. Merritt had been resolved. Additionally, Ms. Homewood performed a stream
determination on UT -2. In response to Ms. Homewood's stream call on UT -2 and correspondence dated
May 25, 2017, DMS submitted a revised close-out report on August 3, 2017 for review. An asset map
and asset table are attached.
As a result of Ms. Merritt's review of the revised close-out request, the following mitigation is approved:
