Dear Mr. Bngman
The Division of Water Resources (the Division) became aware of an error in your recently issued permit
(June 5, 2017). Review of the components list on the Supplement to Permit Cover Sheet indicates incorrect
components have been listed for the facility. Confirmation of the correct components was verified with the
Winston Salem Regional Office staff.
Accordingly, we hereby forward the modified permit pages to amend your permit changing the components
list on the Supplement to Permit Cover Sheet. Please msert these change pages into your existing permit
and discard the old pages. We regret any inconvenience this causes your organization.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements
to obtain other permits which may be required by the Division of Water Resources or any other Federal,
State, or Local governmental permits that may be required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked, and as of this issuance, any previously issued permit bearing this number is
no longer effective. Therefore, the exclusive authority to operate and discharge from this
facility arises under the permit conditions, requirements, terms, and provisions included
herein.
The Town of Yanceyville
is hereby authorized to:
1. Continue to operate an existing 0.45 MGD wastewater treatment system that
includes the following components:
• Mechanical Bar Screen
• Manual Grit Chamber
• Influent Pump Station
• Dual Sequencing Batch Reactors
• Post Flow Equalization Chamber
• Effluent Flow Meter
• Chlorine Contact Chamber
• Dechlorination Tank
• Aerated Sludge Lagoon
The facility is located at the Town of Yanceyville WWTP, at the end of Pine Street,
south of Yanceyville, in Caswell County.
2. After receiving an Authorization to Construct (ATC) permit from the Division,
construct and operate additional facilities needed to meet the 0.60 MGD effluent
limitations and conditions in the permit. After submitting an engineering
authorization (EA) documenting compliance with the ATC, operate at 0.60 MGD.
3. Discharge from said treatment works via Outfall 001, at the location specified on
the attached map into Country Line Creek [22-56-(3.7)] currently classified C
waters in subbasin 03-02-04 [HUC: 03010104021 of the Roanoke River Basin.
