Box 7728 Rocky Mount, NC 27804-7728 Subject: White Oak Manor of Waxhaw REQUEST FOR MORE INFORMATION Dear Mr. Cox, The Division of Water Quality (DWQ) received your application for a 401 Water Quality Certification for the subject project on September 8, 2008. A Request For More Information/Hold Letter was sent to you on October 3, 2008. Additional information was received on November 3, 2008. Barry Love of this office has reviewed the submitted information. Based on this review, 5 copies of the following information/comments are still needed: o A culvert cross-section, profile and plan view showing that the culvert is properly buried in the streambed to allow passage of aquatic life and maintenance of a low flow stream channel. One culvert should be lower (buried in stream bed 20% of culvert diameter or one foot for a 48 inch or greater culvert) than the other to maintain the low flow stream channel through one while the higher can be accessed during higher storm flows. The lower culvert should maintain the stream pattern, profile and dimension. o The crossing area has been moved to address avoidance and minimization. Please address any other efforts taken to reduce the length of impact such as the use of headwalls or reduced riprap length. o Just below the crossing is a sharp bend in the stream. This appears to be in the riprap apron area. Address any efforts that will be taken to keep this area stable. Show the area covered by the apron. Also describe how the riprap will be placed. Any in-stream riprap must be embedded in the stream channel without the use of matting and minimizing the length of impact as much as possible. If we do not hear from you in 30 calendar days, we will assume that you no longer want to pursue this project, and we will consider the project as returned. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested in your application are not authorized at this time by DWQ.
