JAY ZRVIMERMAN Director RECEIVEDINCDENWR SEP 01 2017 Water Quality Permitting Section You are requested to attend a Show Cause Meeting to be held at the Asheville Regional Office (2090 U.S. 70 Highway), on September 21, 2017 at 10 am. This meeting will allow you an opportunity to explain the circumstances involved in the wastewater bypass event or events occurring at the Crossnore wastewater treatment plant. Specific items to be discussed: 1. Specific operational issues causing the May 24, 2017 bypass and any other previous bypasses. 2. Actions you took as the ORC to avoid and respond to the bypass(es). 3. Reporting related to the bypass(es) including the June 21, 2017 NOV response. This meeting has been prompted by information obtained during a complaint investigation conducted by Daniel Boss on May 24, 2017. Pursuant to 15A NCAC 08G.0801, disciplinary actions against a certified operator may result from any of the following actions of the operator: a) practicing fraud or deception in the performance of duties; b) failure to use reasonable care or judgment in the performance of duties; c) failure to apply their knowledge or ability in the performance of duties; d) incompetence or the inability to perform duties; You are asked to bring all pertinent information, including but not necessarily limited to, ORC logs, data sheets, maintenance records, field notes, letters to the owner that demonstrate you have met the responsibilities of an operator in responsible charge (ORC) as described in the Rules (15A NCAC 08G.0204). Of specific interest is documentation supporting your effort to: 1. Properly operate and maintain all systems of treatment and control used to achieve compliance. Take all reasonable actions to minimize or prevent any discharge in violation of the permit. 3. Notify the DWR within 24 hours of discovering the bypass.
