EXPRESS REVIEW PROGRAM NOTIFICATION
Dear Mr. Smith:
On April 11, 2006, the Division of Water Quality (DWQ) issued your Modification to 401 Water
Quality Certification and Authorization Certificate per the Neuse River Buffer Protection Rules
as required to construct the residential subdivision on the subject property. On August 4, 2006,
DWQ received your Notice of Construction; however, within the approval letter that you
received, there is also a condition that reads:
1. Certificate of Completion
Upon completion of all work approved within the 401 Water Quality Certification or
applicable Buffer Rules, and any subsequent modifications, the applicant is required to
return the attached certificate of completion to the 401 Oversight/Express Review
Permitting Unit, North Carolina Division of Water Quality, 1650 Mail Service Center,
Raleigh, NC, 27699-1650.
This is a part of the Express Review Program that is required to ensure compliance during
construction and post construction. Therefore, if you have completed your project, you are
required to notify this office. in writing by January 1, 2009. If you do not notify this office in
writing and construction is complete, you are in violation of the Water Quality Certification
Rules as required by 15A NCAC 2H.0507(c).
This letter is to notify you and make you aware of the conditions that are in the Water Quality
Certification issued within the Express Review Program. Please call Lia Myott Gilleski at (919)
