Dear Ms. Parkerson:
Per the terms of your NPDES permit, you are required to submit DMRs each month, with the due date for
each DMR being the last day of the following month. To date, the Division has not received the followmg
DMRs for your facility:
• March 2017 — due no later than April 30'h
• April 2017 — due no later than May 31'
Failure to submit DMRs is a violation of your permit, subjecting you to a possible assessment of civil
penalties. To reduce the likelihood of additional enforcement action, you must submit the missing DMRs no
later than September 11, 2017.
