EXPRESS REVIEW PROGRAM NOTIFICATION
Dear Mr. Perry:
On June 6, 2006, the Division of Water Quality (DWQ) issued your 401 Water Quality Certification and
Authorization Certificate per the Neuse River Buffer Protection Rules as required to construct the
residential subdivision on the subject property. Within the approval letter that you received, there are
conditions that read:
1.. Notification of Construction
The applicant and/or authorized agent shall contact the DWQ Express Review Program in writing
at the letterhead address within ten (10) days of the commencement of construction.
2. Certificate of Completion
"Upon completion of the project, the Applicant shall complete and return the enclosed
"Certificate of Completion" form to notify NCDWQ when all work included in the §401
Certification has been completed. The responsible party shall complete the attached form and
return it to the 401/Wetlands Unit of the NC Division of Water Quality upon completion of the
project. Please send photographs upstream and downstream of each culvert site to document
correct installation along with the Certificate of Completion form." Please return the attached
certificate of completion to the 401 Oversight/Express Review Permitting Unit, North Carolina
Division of Water Quality, 1650 Mail Service Center, Raleigh, NC, 27699-1650.
3. Resolution of the Notice of Violation
The restored buffer shall be monitored for three (3) years. Two (2) copies of an annual report
shall be submitted-one to the attention of the DWQ Raleigh Regional Office, and one to the
attention of Ms. Cyndi Karoly, DWQ Central Office.
This is a part of the Express Review Program that is required to ensure compliance during construction
and post construction. Therefore, if you have begun construction of your project or have completed your
project, you are required to notify this office in writing by January 1, 2009. If you do not notify this
office in writing and construction has started, you are in violation of the Water Quality Certification Rules
