Thanks for getting back, know you have been busy. Attached is the revised culvert design which elevates one of the two
72" RCPS by 18" to allow for baseflow through one culvert and storm flows through the other, as we discussed. Also
attached is the original plan/profile for this crossing for reference.

Kevin,
I apologize for the delay in my response. The 401 WQC expiration is tied to the Individual Permit
expiration. New buffer calls would not be required, but you would probably need to modify the mitigation
phasing schedule. BTW — I have not received the revised culvert design to ensure that the pattern, dimension
and profile of the stream is maintained to issue the modification that was originally requested back in
March. Please let me know the status of this information.

Hey Sam/Jennifer,
The Beth Page Individual Permit is set to expire December 19, 2019, with the intent of all permitted impacts to streams,
wetlands, and Neuse Buffers completed by that time. However, given that the overall project is only halfway built -out at
this time, all permitted impacts may not be complete before the expiration date. General Condition (1) of the IP
states: "...If you find that you need more time to complete the authorized activity, submit your request for a time
extension to this office for consideration at least one month before the above date is reached."
Fortunately/Unfortunately, I haven't been around to know how the process works for requesting an extension of an IP
expiration date and seeking your guidance on a couple questions:
1) Will we need to re -verify the JD and Neuse Buffer calls at this time or before?
2) There will be no other request for additional impacts, would this request for additional time require the project to
go back on Public Notice?
3) Would all mitigation requirements need to be met prior to December 19, 2019?
4) Any other thoughts?
