REQUEST FOR MORE INFORMATION
Dear Mr. Coleman:
On September 22, 2008, the Division of Water Quality (DWQ) received your application dated
September 17, 2008, to fill or otherwise impact 0.0949 acres of 404/wetland, 30 linear feet of
intermittent stream, and 900 square feet of Zone 1 Neuse River basin protected riparian buffers,
and 3,020 square feet of Zone 2 Neuse River basin protected riparian buffers, to construct the
proposed mixed use development at the site. The DWQ has determined that your application
was incomplete and/or provided inaccurate information as discussed below. The DWQ will
require additional information in order to process your application to impact protected wetlands
and/or streams on the subject property. Therefore, unless we receive five copies of the additional
information requested below, we will place this project on hold as incomplete until we receive
this additional information. If we do not receive the requested information, your project will be
formally returned as incomplete. Please provide the following information so that we may
continue to review your project.
Additional Information Requested:
1. Is this a contaminated site or a Brownfields site? If so, please discuss the nature of and
current status for the site (including narrative regarding whether it is soil and
groundwater contamination, groundwater contamination only, or soil contamination only.
2. Please complete Section VI, Item No. 4 on the application.
3. Please provide cross section details showing the provisions for aquatic life passage.

4. Please locate all isolated or non-isolated wetlands, streams, and other waters of the State
as overlays on the site plan.
5. Please locate all of the protected riparian buffers as overlays on the site plan, clearly
showiniz Zone 1 and Zone 2.
6. Per the requirements of the Neuse Riparian Buffer Rule, you must show that this site
meets diffuse flow requirements with a level spreader or other BMP per Chapter 8 of the
BMP Manual (see http://h2o.enr.state.nc.us/su/bmp forms htm).
7. Please submit a Level Spreader Supplement Form with all required items for each
proposed level spreader (see http://h2o.enr.state.nc.us/su/bmp forms htm).
8. Please show contour lines down slope of the level spreader to show that it has been
correctly sited per the BMP Manual.
9. Please delineate drainage areas on the plan and provide an inventory of imperviousness
surfaces in each drainage area. The inventory should include all proposed building
footprints, roads, driveways, sidewalks, gravel-surfaced areas, amenity areas, etc. A
Stormwater Management Plan (SMP) must be provided for any drainage areas that
exceed 24 percent imperviousness. For each BMP, provide a completed BMP
Supplement Form with all the required items (see
http://h2o.enr.state.nc.us/suibmp forms htm) for each ro osed BMP. Per NCAC 2B
15A: 02H.1000, a "drainage area" is defined as "the entire area contributing surface
runoff to a single point."
10. Please resubmit 5 copies of your information on the March 2005 PCN form or a more
recent version. You can download this format the following web address: (see

11. Please provide stream determination documentation on the site (upstream and
downstream photos and DWQ stream identification forms). According to the Johnston
County Soil Survey, you have more streams and buffers on your property.
12. Please provide stormwater BMP bypass details and include square footage on PCN
application impacts.
13. Please note that the correct General Certification (GC) No. you are applying under is
GC3705.

Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian
