Deaz Sir:
The North Carolina Departrnent of Transportation (NCDOT) proposes a modification to
the existing Individual Section 404 Permit and 401 Certification for the R-2250 project
(WBS 34411.1.2). The R-2550 project includes construction of the Greenville Southwest
Bypass from south of Old US NC 11 to US 264 in Pitt County. The project construction
contract was awarded in May 2015 and construction began when a11 permits were
obtained in August 2016. Enclosed with this modification letter aze updated permit
drawings and an updated summary impact table.
Modification
This modification is being requested due to the need for extension of Service Road 5 on
Permit Impact Sheet No. 37 at SRS-Sta. 11+26 to 12+17. This location is illustrated as
Site 12 in the attached drawings and summary impact table. This extension is necessary

to provide equal to or better access to parcels being impacted by the roadway project.
There will be no stream or buffer impacts at this location.
The projecYs permit impact summary table and drawings have been updated to include
the additional 0.04 acre of wetland impacts. These areas of wetland impact were
considered total takes
and aze shown as fill beyond the right of way line on the attached plan sheets.
Mitigation
Compensatory Mitigation will not be required for the impacts noted within this
modification.
Regulatory Approvals
Section 404: We are hereby requesting the modifications described above for the USACE
Individua1404 Permit signed August 8, 2016 (SAW-2006-40466), for the above-
described activities.
Section 401: We aze hereby requesting a modification to the 401 Water Quality
Certification from the NCDWR issued July 7, 2016 (DWR No. 20160191). In
compliance with Section 143 2153D(e) of the NCAC, we will provide WBS 34411.1.2 to
debit $570.00 to act as payment for processing the Section 401 permit modification.
