References: Section 404 and 401 Individual Permit Application requested March 28, 2016 Section 404 Individual Permit issued August 8, 2016 (SAW-2006-40466) Section 401 Water Quality Certification issued July 7, 2016 (20160191) Dear Sir: The North Carolina Department of Transportation (NCDOT) proposes a moditication to the existing Individual Section 404 Permit and 40l Certification for the R-2250 project. The R-2550 project includes construction of the Greenville Southwest Bypass from south of Old US NC 11 to US 264 in Pitt County. The project construction contract was awarded in May 2015 and construction began when all permits were obtained in August 2016. Enclosed with tl�is modification letter are updated permit drawings and an updated summary impact table. Modification Tk�is modification is being requested due to the need for extension of Service Road 5 on Permit impact Sheet No. 37 at SR5-Sta. I]+26 to 12+17. This location is illustrated as Site 12 in the attached drawings and summary impact table. Tl�is extension is necessary to provide equal to or better access to parcels being impacted by the roadway project. Mitigation Compensatory Mitigation will not be required for the impacts noted within this hnodification� OR Compensatory mitigation will be required for the additional 0.04 acre of wetland impact noted within this modification. Tl�is impact increases the total project wetland impacts to 037 acre. The Department has acquired the compensatory mitigation for unavoidable impacts from the North Carolina Department of Environmental Quality (NCDEQ)-Division of Mitigation Services (DMS). A revised DMS letter is attached for the additional 0.04 acre of wetland impact. Regulatory Approvals Section 404: We are hereby requesting the modifications described above for the USACE Individua1404 Permit signed August 8, 2016 (SAW-2006-40466), for the above-described activities. Section 401: Wc are hereby requesting a modification to the 401 Water Quality Certification from the NCDWR issued July 7, 2016 (DWR No. 20160191). In compliance with Section 143-2153D(e) of the NCAC, we will provide $570.00 to act as payment for processing the Section 401 permit �nodification� A copy of this modification request and its distribution list will be posted on the NCDOT website at: httns://connect.nedot.�ovhesources/Environmental.
