Willow Oak Mobile Home Park WWTP, Rockingham County
Dear Mr. Radford:
On July 17, 2017, Paul DiMatteo of the Division of Water Resources ("Division"), Winston-Salem
Regional Office conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment
system. Paul Smith, Operator in Responsible Charge (ORC), was present for the inspection. An inspection
summary follows and an inspection report is attached for your records.
Site Review
1. Permit
The current permit became effective on July 1, 2016 and expires on April 30, 2021. A copy of the
current permit was not available for review — one should be kept for inspection. Permitted
components include a bar screen, facultative lagoons and alum feed tanks.
2. Facility Site Review
The facility was as described in the permit. Flow is low compared to the permitted amount
(permitted for 22,000 gpd; average reported flow over the last 2 years is about 150 gpd).
3. Operation and Maintenance
The operator or a properly designated backup visits the site as required. Visitation logs were
current and available during the inspection.
The 4th chamber of the alum feed system was observed to be leaking on the lagoon -facing side. A
follow-up visit was conducted on July 31 to verify that the leak had been plugged, but the ORC
mentioned additional maintenance may be needed.

4. Flow Measurement
The permit requires instantaneous flow measurement influent or effluent. Flow is measured at the
influent.
5. Effluent/Receiving Waters
The effluent is discharged into Little Troublesome Creek. The outfall was not accessible during the
inspection due to the lack of a path and swampy ground.
Documentation Review
6. Records/Reports
Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs
include all permit parameters. Copies of DMRs, operator visitation logs, lab reports and Chain -of -
Custody forms were available for review during the inspection.
7. Self -Monitoring Program
A review of DMRs from May, 2016 through May, 2017 showed no violations. A comparison of
submitted DMR data with bench sheets did reveal one minor error on the August, 2016 DMR. Mr.
Smith said he would more carefully check that the data has been transferred correctly before
submittal.
8. Laboratory
Field parameters are measured by the ORC under certification # 5169 (Smith Industries). All effluent samples
are taken from the aerobic digester. All other parameters are measured by Pace Analytical under
certification # 633. Chain -of -custody forms were complete and available for all parameters measured off
site. Records for parameters measured on-site were complete.
9. Sludge Handling Disposal
Sludge is removed from the plant as needed.

There was one minor mistake noted on the August. 2016 DMR. Lab report for Nitrogen -
Ammonia was 3.9 mg/I - DMR reported 3.0 mg/I. Mr. Smith said that he would be more
careful and double check values before submitting.

Comment- In the lower left corner of the bar screen there appeared to be one vertical bar seament
missing. This appeared to lead to channeling of debris and water in the chamber
immediately prior to the bar screen. According to the ORC this is necessary to prevent the
bar screen from clogging and overflowing. The plant is only required to be visited once per
week.

Comment: Mr. Smith reported that temperature was recorded using an infrared thermometer. Field lab
rules state that Infrared cannot be used for temperature readings reported on the DMR. Mr
Smith was informed of this requirement, and said that he would obtain and use a proper
thermometer.
