Dear Mr. Miller:
On October 13, 2008, the Division of Water Quality (DWQ) received the PCN application for the above
referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. Please provide the following so that we may continue to
review your project.
Additional Information Requested:
N Incorrect and/or missing wetland, intermittent-perennial stream and/or buffer impacts.
The total stream impacts noted on Pages 8 and 9 of 12 appear to be posted incorrectly. Page 8
and 9 shows total stream impact at 7,900 linear feet; however the figures shown equal 7,150
linear feet. Please provide five (5) copies of corrected information.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and that you remain interested in continuing to pursue permitting of your project and will be
providing the DWQ the requested information at a later date. Please contact this office in writing. If we
do not hear from you within three weeks, we will assume that you no longer want to pursue this project
and we will consider the project as returned.
