1.2 Point of Responsibility
Burl Domecq, Plant Superintendent is responsible for maintaining and
implementing this SWPPP.
The responsible person listed above will identify the facility stormwater pollution prevention
team, and assign responsibilities, as necessary, to accomplish the requirements of this plan.
The Pollution Prevention Team is identified below.

Plant Foreman facilities with regard to stormwater pollution.
Each member of the stormwater pollution prevention team must have ready access to either an
electronic or paper copy of applicable portions of the SWPPP.

1.3 General Location Map
Appendix A provides a general location map (U.S. Geological Survey (USGS) 7.5 minute
quadrangle map) showing the location of the facility and all receiving waters for stormwater
discharges.

Appendix B provides a site map of the facility boundary and size of the property in acres and
locations of the activities listed in Table 1 below.
SECTION 2: POTENTIAL POLLUTANT SOURCES
2.1 Associated Pollutants
The site, being an asphalt plant, stores materials needed to produce hot -mix asphalt concrete.
Table 1 below lists the associated pollutants on-site. These activities are shown on the site map
in Appendix B.

The facility utilizes twenty (20) dump trucks for product hauling, which are maintained on-site.
2.2 Spills and Discharges
Appendix C provides a list of spills or leaks of pollutants that have occurred at the facility during
the three (3) previous years and the corrective actions taken to mitigate spill impacts.
2.3 Non-Stormwater Discharge Documentation
Appendix D provides the visual assessment form used during the initial evaluation of non-
stormwater discharges at the facility. Information gathered included: drainage point ID,
drainage point type, and dry -weather flow. Outfalls identified with non-stormwater flow were
evaluated further.

SECTION 3: STORMWATER CONTROL MEASURES
3.1 Structural Controls
The structural controls shown on the Site Map (Appendix B) are described below. These
controls are used to minimize the exposure of the activities identified in Section 2.1.
• (S1) AC -20 Liquid Asphalt: Routine Inspections; Drip Pans to catch minor oil leaks
• (S2) Heating Oil: Routine Inspections; Drip Pans to catch minor oil leaks; Routine
Pumping Out
• (SA, RA, SH) Bulk Stockpiles: Placement of material at elevated portions of the site;
Routine Inspections of drainage areas.
3.2 Good Housekeeping
• Daily yard maintenance, and enhancement of grass and vegetation in buffer
areas. Keep site clean of mud and graded for proper drainage.
• Keep outside areas neat, orderly and free of trash.
• Review Environmental guidelines for the proper handling of waste and refuse.
• Discuss labeling of materials and proper storing requirements of those that are
environmentally hazardous.
• Review procedures for both routine and emergency clean-ups.
• Discuss Plant Inspections, Check Lists and What to look for.
3.3 Recommended Best Management Practice
A formal Best Management Practices policy should be developed by the Plant Staff and
implemented through an employee training program. Particular items that to be included are:
• Identify Oil leaks on or around the AC -20 tank and any appurtenances that need to be
cleaned and repaired.
• A written policy for proper methods of liquid material handling should be completed.
This could be incorporated into an employee training program.
• Structural examinations and leakage tests need to be performed on the containment
structure(s) every 2-3 years.
• A written proposal for the proper disposal of any fuel or oil laden stormwater as
collected on the drums, sand or soil, should be developed and incorporated into a
training program.

3.4 Spill Prevention and Response
• Review potential spill conditions and causes (i.e. lack of attention to tasks, faulty
equipment, unusual weather conditions).
• Discuss emergency spill response procedures and where telephone numbers are posted.
• Copy and review with all employees the requirements set forth in the current Spill
Prevention Control and Countermeasures Plan (SPCC) document. This should be done a
minimum of once each year.
• Discuss labeling of all chemicals and materials containers for quick identification and
reference should an emergency occur.
• Assign and review with employees specific clean up and inspection duties.
3.5 Management of Runoff
Sources of pollution on this site are primarily from oil and fuel storage facilities as well as minor
potential from some of the Plant Process equipment. Sand and fine rock screenings also have
the potential to wash off the site and cause ditches, streams, and storm pipes to clog or
increase turbidity in fish inhabited streams.
This plan calls for the use of sheet flow and shallow swales to divert runoff to existing perimeter
ditches, internal to the property boundaries that convey that runoff to existing sediment basins.
Cleanup of said ditches and basins are proposed as a part of the upstart of this facility as well as
regular inspection and maintenance. Existing Basins will be used for Erosion & Sediment control
during construction of the plant site improvements. Upon completion and stabilization of the
site, basins will be left in place and any skimmers or temporary measures removed.
3.6 Employee Training
Personnel at all levels of responsibility will be trained in the components of the SWPPP and the
SPCC. At a minimum, training will be provided on an annual basis. New personnel will receive
training promptly upon assignment. All training will be documented.
SECTION 4: INSPECTIONS
The facility will conduct the following inspections:
Routine facility inspections;
Quarterly visual assessment of stormwater discharges; and
Comprehensive site inspections.

Routine facility inspection
Plant Superintendent, or specific designee will conduct routine facility inspections, using the
form in Appendix E. Each structural control measure and activity area are visually inspected on
a minimum annual basis, with more frequent inspections conducted as necessary to minimize
the potential for a spill or release.
Quarterly visual assessment
Plant Superintendent, or specific designee will conduct quarterly visual assessments, using the
form in Appendix D. Each drainage point is visually inspected on a quarterly basis. At least one
inspection each year will be conducted when a stormwater discharge is occurring.
Comprehensive facility inspection
Plant Superintendent, or specific designee will conduct comprehensive facility inspections,
using the form in Appendix F. Each structural control measure, activity area, and drainage point
are visually inspected on an annual basis.
SECTION 5: SWPPP CERTIFICATION
The following certification statement must be signed and dated by the responsible signing
official.
This certification must be re-signed in the event of a SWPPP modification in response to a Part
3.1 trigger for corrective action.

direction or supervision in accordance with a system designed to assure that qualified
personnel properly gathered and evaluated the information submitted. Based on my inquiry of
the person or persons who manage the system, or those persons directly responsible for
gathering the information, the information submitted is, to the best of my knowledge and
belief, true, accurate, and complete. I am aware that there are significant penalties for
submitting false information, including the possibility of fine and imprisonment for knowing
violations.

SECTION 6: SWPPP MODIFICATIONS
This SWPPP is a "living" document and will be modified and updated, as necessary. Actions
which can trigger the need for a plan amendment include:
• Facility modifications which may affect runoff quality or quantity
• New operations
• New materials
• New maintenance procedures
All SWPPP modifications, will be kept in the log (Table 2) with a description of the modification,
the name of the person making it, and the date and signature of that person.
