Storm event during Hurricane Matthew (October 2016) resulted damage to 2@ 10 feet wide pipe structures that
are 119 feet long. The storm event resulted in failed pipe structures, contributing to a sink hole in the road above
the structures. The purpose of this project is to removed a portion of the road bed and failed pipe structures and
then construction of a Class II rip rap open channel.

Contractor will conduct the following: 1. Removal of "dry" roadbed above open water; 2. Removal of "wet or
I submerged" roadbed below water surtace, 3. Remove damaged pipe structures and 4. Construct Class II rip rap
lined open channel. Equipment MAY include: excavator, long-arm stick excavator, dredge bucket, vacuum system,
etc.

This PCN form covers the following: 1. Removal of "dry" roadbed above open water; 2. Removal of "wet or
submerged" roadbed belowwater surface, 3. Remove damaged pipe structures and 4. Construct Class II rip rap
lined open channel. The NCDOT will install a crossing structure (likely a one-span bridge) in 2018. The
Department has not determined at this time if there will be additional stream, wetland, buffer or open water impacts
associated with the crossing structure.

1a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:*
The Department will remove by excavation only the minimum amount necessary of fill material to construct the
Class II rip rap lined open channel. Removal of material will be restricted to within 20 feet of the toe of existing
slope or 10 feet from the pipe structure, whichever is less. The contractor may temporarily store excavated
material on-site, but will eventually be required to remove the material from the project site.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:*
The Department has had several conversations ond meetings with the regulatory agencies regarding this project
since October 2016. The regulatory agences requested that the Department evaluate all options to remove both
"dry" and "wet or submerged " portions of the fill slope and the damaged pipe structures. On June 13, 2017
NCDOT Dlvision 5 staff and Dewberry staff inet with the USACE, NCDENR-DWR and NCWRC to evaluate all
possible construction options. See the attached Redwood Road Study Final Report. After presentation of the
study alternatives and analysis, the consensus of opinion among those present and participating by phone was
that the wet excavation method (Alternate III) detailed in this report is the recommended alternative for
development of the pipe replacement project for construction and permitting. The primary environmental issue of
concern e�ressed by the permitting agencies present is control of additional turbidity potentially generated by
construction activities. The method of control during construction is to be turbidity curtains and turbidity
monitoring.
