Explain the purpose of the proposed project: The reason for the project is that portions of the interlocking concrete block are failing, resulting in localized bank erosion. If not repaired, the channel banks will continue to erode potentially threatening surrounding infrastructure. B.3e. Describe the overall project in detail, including the type of equipment to be used: Areas of failing interlocking concrete block will be removed and the stream banks re -graded. The interface of the block with natural ground will be grouted to provide additional structural strength. Boulder vanes will be installed in the repair areas to divert erosive flows away from the channel banks. A floodplain bench will be constructed at the downstream location. A small area of accumulated sediment on the designed bench will be removed to restore the designed capacity to the channel. This work will occur above the OHWM. Track excavator and dump trucks will be used during construction. All excess excavated material will be placed directly into dump trucks and hauled from the site or disposed of outside the channel. 8c. What source(s) did you use to make the floodplain determination? HAMMTON S__y NCNHDE-3478 The North Carolina Natural Heritage Program (NCNHP) appreciates the opportunity to provide information about natural heritage resources for the project referenced above. A query of the NCNHP database, based on the project area mapped with your request, indicates that there are no records for rare species, important natural communities, natural areas, or conservation/managed areas within the proposed project boundary. Please note that although there may be no documentation of natural heritage elements within the project boundary, it does not imply or confirm their absence; the area may not have been surveyed. The results of this query should not be substituted for field surveys where suitable habitat exists. In the event that rare species are found within the project area, please contact the NCNHP so that we may update our records. The attached 'Potential Occurrences' table summarizes rare species and natural communities that have been documented within a one -mile radius of the property boundary. The proximity of these records suggests that these natural heritage elements may potentially be present in the project area if suitable habitat exists and is included for reference. Tables of natural areas and conservation/managed area within a one -mile radius of the project area, if any, are also included in this report. Please note that natural heritage element data are maintained for the purposes of conservation planning, project review, and scientific research, and are not intended for use as the primary criteria for regulatory decisions. Information provided by the NCNHP database may not be published without prior written notification to the NCNHP, and the NCNHP must be credited as an information source in these publications. Maps of NCNHP data may not be redistributed without permission. The NC Natural Heritage Program may follow this letter with additional correspondence if a Dedicated Nature Preserve (DNP), Registered Heritage Area (RHA), Clean Water Management Trust Fund (CWMTF) easement, or Federally -listed species are documented near the project area. If you have questions regarding the information provided in this letter or need additional assistance, please contact Rodney A. Butler at rodney.butler0ncdc r.gov or 919.707.8603. Any utility locations shown on these plans are approximate. The Contractor shall have all utilities within the project limits located and marked prior to beginning construction. The Contractor will be responsible for the repair of any utilities damaged during construction. 2. Contractor shall only use construction site entrances shown on the plans. 3. Temporary staging areas shall be established as directed by the Engineer. Install silt fence around the down- slope perimeter of the staging/stockpile areas to avoid sediment runoff. 4. Construction shall proceed upstream to downstream. 5. Begin construction of the rock vane at station 10+50 (RV1) by accessing from the western stream bank and bench area. Remove failing interlocking block on the east bank at the install location of RV1 to allow placement of boulders to tie into exposed bedrock seams. Boulder placement may need to vary from the orientation shown in the typical detail to achieve the target elevations and vane slopes. After boulders have been placed and approved by the Engineer, Contractor shall use concreate grout to secure edges of interlocking block with the boulders and bedrock, sealing the structure and locking the concrete blocks into place. Contractor may place stream gravel and alluvium over concrete after curing for 48 hours. 6. Contractor shall remove failing interlocking block from approximately station 15+50 to 17+00 to allow for excavation of the bankfull bench and installation of RV2 and RV3. 7. Excavate the bankfull bench from station 15+50 to 17+00 as shown in the plans and haul soil material to off- site disposal area. 8. install RV2 and RV3 according to the plans. Footer rocks may be required for RV2 and RV3 depending on the depth to bedrock. 9. Install coir fiber matting along newly graded bank from station 15+50 to 17+00. 10. Place concrete grout as shown on the plans to lock in place the exposed edges of interlocking block at approximate station 15+50 and 17+00. 11. Place concrete grout patch over section of partially failing interlocking block at approximate station 17+50, on the southern bank downstream of the rail road bridge. 12. Contractor shall apply mulch, temporary seed, and permanent seed to exposed soil areas immediately after completion of construction activities in those areas. 13. Ensure that all trash and excess materials have been removed from the site prior to demobilization. REVISIONS NO. DESCRIPTION ENGR.
