Additional information was requested in order to process our application. The additional information requested is as follows. Please provide a more detailed profile view and cross section of the proposed stream culverts. Please also submit detailed drawings indicating pipe burial. For the permanent crossing, the Division will require that the natural streambed be routed through only one of the proposed culverts and the other culvert be constructed as a flood plain pipe and installed at bankfull elevation or a sill provided on the pipe inlet. Flood plain benches shall be installed at both ends of the pipe to establish natural channel dimensions above and below the crossing. This information must all be clearly detailed on the plans and resubmitted to the Division for approval. New sheets have been created for each stream crossing showing plan and profiles for the existing and proposed condition. The culvert has been changed to a box section due to sanitary sewer constraints which limit raising the 54" pipe that was previously proposed. 2. Please note that the due to the length of time the temporary crossing will be installed, the temporary pipe will require proper burial into the stream bed to allow for aquatic life passage. A site visit conducted on August 11, 2008 noted a large bedrock grade control structure in the stream at approximately the location of the culvert inlet. Please clarify how this structure will be addressed during installation of the culverts. Please also note that although the Division typically requires culvert burial, we prefer to avoid blasting into bedrock in or directly below the streambed for the purposed of pipe burial. You should identify the locations of all bedrock within the area of the culvert installation. The rock outcropping will be constructed over. This is indicated on the plan for this crossing. 4. The site visit also noted a sharp turn within the stream channel directly downstream of the culvert outlet. Additional impacts may eventually be necessary to address the shear stress upon this streambank. The permit issued to you will require a preconstruction meeting, as well as a final approval after installation. Additional stabilization work may be necessary at those times. You may address this situation now if you choose to avoid possible future repairs and permit modifications. The construction sequence indicates a pre- and post- construction meeting with the engineer and NCDENR. 5. Per the requirements of GC 3705, please enumerate all temporary impacts (in addition to permanent impacts already indicated) to the stream channel and any wetland areas. Impacts from the construction of a floodplain bench would be considered temporary impacts. Temporary impacts also include areas of disturbance necessary for proper pipe installation, as well as any impacts from temporary erosion control measures expected to be installed within any jurisdictional areas. Please note that temporary impacts should be minimized to the maximum extent practical. Attached is a revised copy of the pertinent PCN sheet showing the verified impacts as well as an impact map showing the areas in question. 6. Per the requirements of GC 3705, all work be done "in the dry". Therefore, dewatering methods will be necessary to temporarily dewater the stream channel during pipe installation. Please provide a construction sequence that details the method of dewatering to be utilized at the site and the sequence of construction events to be followed to ensure compliance with this condition. The plan & profile sheets for each crossing show diversions necessary to reroute the stream flows during construction. 7. Per the requirements of GC 3705, this project must comply with Stormwater Management Plan (SMP) Requirements for Applicants Other Than the North Carolina Department of Transportation, available at: http://h2o. enr. state.nc.us/ncwetlands/documents/SMPReguiremtnstforApplicanstotherthanNCDO T2.pdf. It appears that this project will be subject to another state implemented program. Please verify that you have, or intend to, submit a stormwater management plan under that program.
