You have our approval, in accordance wi[h [he conditions listed below, for the following additional impac[s for Ihe
purpose of replacing Bridge 232 in Johns[on County:

Total Additional Stream Impact for Projeck 44 linear feet.
The projec[ shall be constructed in acwrdance with your application received July 28, 2016 and modification
request received July 27, 2017. After reviewing your application, we have decided that these impacts are covered by
General Water Quality Certification Number 4085. This certification corresponds [o the Nationwide Permit 3 issued
by the Corps of Engineers. This approval is also valid for the Neuse Riparian Buffer Rules Q SA NCAC 2B .0295).
In addition, you should acquire any other federal, state or local permits before you proceed with your project
including (but not limited to) Sediment and Erosion Control. Non-Discharge and Water Supply Watershed
regulations. This approval will expire with the aceompanying 404 permit.
This approval is valid solely for [he purpose and design described in your application (unless modified below).
Should your project change, you must notify the NCDW R and submit a new application. If [he property is sold, the
new owner must be given a copy of this Certification and approval letter, and is thereby responsible for complying
with all the conditions. If total wetland fills for this project (now or in the fumre) exceed oce acre, or of total
impacts [o streams (now or in the fo[ure) exceed I50 linear fee[, compensatory mitigation may be required as
described in ISA NCAC 2H .0506 (h) (6) and (7). Additional buffer impacts may require compensatory mitigation
as described in 15A NCAC 2B.0233. For this approval ro remain valid, you must adhere ro the conditions listed in
the attached cenification and any additional conditions listed below.

I. All conditions in the Original 401 Certiflcation for this project, dated August 8, 2016, still apply.

If you wish to contest any staremen[ in the attached Certifica[ion you must file a peti[ion for an administrative
hearing. You may ob[ain the pe[i[ion form from [he office of Administrative hearings. You must file [he pe[ition
with the office of Administrative Hearings within sixty (60) days of receipt of this notice. A petition is considered
filed when i[ is received in the office of Administretive Hearings during normal oftice hours. The Oftice of
Administrative Hearings accepts filings Monday through Friday between the hours of S:OOam and S:OOpm, except
for official state holidays. The original and one (1) copy of the petition must be filed with the Office of
Administrative Hearings.
The peti[ion may be faxed-provided Ihe original and one copy of [he document is received by [he Office of
Administrative Hearings within five (5) business days following the faxed transmission.
