Folks,
Please accept this application for a 404 Permit and 401 Certification for improvements
to the I-26 and US 74 in Polk County. We are hoping to let this project in September
2017 and complete the improvements in advance of a World Equestrian Games Event to
be held nearby in September 2018.
Please advise if there is information missing or if you have any questions or need
additional information to complete your review of this application. I did not include the
PJD request that was submitted earlier in the year before the site visit, but will provide it
and the new forms if needed — please just let me know.
