JAY ZIMMERMAN Director RECEIVEDINCDEQIDWR jUL Q 6 2017 Water Quality. ion permitting A Compliance Evaluation Inspection was performed on the subject facility by Min Xiao of this office on June 26, 2017. Mr. Matt Smith, Operator in Responsible Charge (ORC), was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and terms & conditions with the National Pollutant Discharge Elimination System (NPDES) permit. The inspection findings are detailed below. The underlined paragraph indicated the specific violation noted during this inspection. I. Permit The current NPDES permit expired on May 31, 2017. The Water Quality Permitting Section has received the renewal application on January 18, 2017. A permit draft has been sent out for public comment. H. Self -Monitoring Program The DMRs were reviewed for the period April 2016 through March 2017. The facility was only found to be non-compliant with monitoring frequency of Temperature in June 2016. Flow, pH, Temperature, and Dissolved Oxygen are measured onsite by the operator. The rest of the parameters are tested by R&A Labs. III. Facility Site Review The 0.025 million gallons per day (MGD) treatment plant consists of the following components: a 9,400 - gallon equalization tank with influent bar screen, dual 105 gallons per minute (GPM) equalization pumps, flow splitter, 25,000 -gallon aeration tank with 320 ft2 of suspended media, clarifier with air-lift sludge return pumps, UV disinfection, effluent flow meter, dual 3,000 -gallon sludge holding tanks, and a 36 kW emergency generator. It was noted the emergency generator had not been functional for a long time. This issue has been mentioned during the inspections of 2015 and 2016, however, the problem hasn't been resolved vet. Per Part II Section C. 2 (Proper Operation and Maintenances and 7 ower Failures) of the NPDES permit the permittee shall at all times properly perate and maintain all facilities and systems of treatment and control which are installed or used by the permittee to achieve compliance with the conditions of this permit and the permittee State of North Carolina I Environmental Quality I Water Resources 450 West Hanes MITI Road, Suite 300, Winston-Salem, NC 27105 Phone- 336-776-98001 Intemet www,ncdenr.gov is responsible for maintaining adequate safeguards to prevent the discharge of untreated or inadequately treated wastes during electrical power failures. Algae was present at the clarifier weir. One of the six UV bulbs was burned out at the time of inspection. It was observed extra UV bulbs were stored onsite. Please replace the UV bulb with a new one. It was suggested the ORC store Calcium Hypochlorite onsite as disinfection chemical in case of UV system failure. IV. Records & Reports The ORC did a good job of keeping the paperwork. A comparison of January 2017 DMR with the field records and laboratory results revealed that all the parameters have been transcribed correctly. V. Flow Measurement The NPDES permit requires continuous flow recording and an annual calibration The flow measuring device was calibrated by R&A Labs on March 17, 2017. VI. Effluent/Receiving Waters The effluent to the Country Line Creek, Class C waters, was clear and no odor was noticed VII. Upstream & Downstream Sampling The location of upstream sampling is at least half mile away from the outfall. The downstream samples are collected from Tom W. Buck Bridge on the Country Line Creek at Highway 57. VIII. Action Items: • Repair the emergency generator • Replace the burned -out UV bulb • Store back-up chemical onsite for disinfection (eg. Calcium Hypochlorite) Please respond in writing within thirty (30) business days after receipt of this Notice, including the plan and schedule for emergency generator repairing.
