JAY ZIMMUMAN hW10r 'REcE1VEDI���E��wE �U�_ Q 6 2011 Watel Qua1lttion pecm+tog Seo On June 22, 2017, Paul DiMatteo of the Division of Water Resources ("Division"), Winston-Salem Regional Office conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment system. Mike Carson, Operator in Responsible Charge, was present for the inspection. An inspection summary follows and an inspection report is attached for your records. Site Review 1. Permit The current permit became effective on April 1, 2017 and expires on May 31, 2021. Permitted components include a recirculating surface sand filter and ultraviolet disinfection. A copy of the current permit was available for review. 2. Facility Site Review The facility is as described in the permit. After initial treatment, 1/3 of the effluent is returned to the head of the plant, and the rest continues to disinfection and then to discharge. The UV system appears to be working properly. Replacement bulbs as well as backup chlorine tablets are maintained on site. 3. Operation and Maintenance The facility appeared to be generally well maintained. Mr. Carson reported that a resident in the community performs general grounds maintenance including raking the sand bed weekly. However, during the inspection the following concerns were noted: a. Operation of the sand filter pump's high water alarm could not be demonstrated due to Mr. Carson's injury and lack of familiarity with the controls. Please ensure the alarm is in proper working order at all times. b. Several concrete access lids to underground tanks were cracked, with one lid being covered by a metal sheet. NC 27105 .1 4. Effluent/Receiving Waters The effluent pipe is located at the bottom of the hill below the sand filter system. The receiving stream is Travis Creek in the Cape Fear River Basin. The effluent appeared clear with no visible solids. 5. Flow Measurement The permit requires weekly instantaneous flow measurement. Flow is estimated based on the community well's water meter. Documentation Review 6. Records/Reports Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs include all permit parameters. Copies of DMRs, operator visitation logs, lab reports and Chain -of -Custody forms were available for review during the inspection. 7. Self -Monitoring Program A review of DMRs from July, 2015 through March, 2017 showed only the following violation: Date Action Number Description 12/31/2015 Notice of Violation NOV-2016-LV-0207 Nitrogen, Ammonia — Monthly Average Exceeded 8. Laboratory Mr. Carson visits the plant weekly and tests the following field parameters: pH, dissolved oxygen and temperature. Mr. Carson's field parameter certification number is 5195. Calibration records for Mr. Carson's meters are maintained with records and reports. Meritech, Inc. analyzes all other parameters for the facility: Biochemical Oxygen Demand, Total Suspended Solids, Ammonia, Total Nitrogen, Total Phosphorus, and Fecal Coliform. Sludge Handling Disposal Pee-Wee's Septic Tank Service removes sludge from the septic tanks approximately every 6 months. Records of this service were available for review during the inspection. The last pump -offs occurred in January 2016, August 2016 and January 2017. We appreciate your efforts in effectively operating and maintaining this treatment system.
