RECAN kcrotary S, JAY ZIMMERMAN fll vdor On June 27, 2017, Paul DiMatteo of the Division of Water Resources ("Division"), Winston-Salem Regional Office conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment system. Marc Nault, Operator in Responsible Charge (ORC), was present for the inspection. An inspection summary follows and an inspection report is attached for your records. Site Review Permit The current permit became effective on June 1, 2017 and expires on May 31, 2022. Permitted components include a 15,000 -gallon dosing tank, recirculating surface sand filter and chlorine disinfection. A copy of the current permit was available for review. 2. Facility Site Review The facility was as described in the permit. The ORC adjusts valves by hand to achieve recirculation of about 1/3 of the effluent after the sand filter. Operation and Maintenance The facility appeared to be well maintained. Mr. Nault reported that the sand in the sand filter was replaced recently and that the sand is raked weekly. Chlorine tablets for disinfection are kept on site and dechlorination tablets are kept with the ORC. 4. Effluent/Receiving Waters The outfall was easily accessible and effluent appeared clear with no noted impact to the receiving stream. Flow Measurement The permit requires continuous flow monitoring at the influent or effluent. Mr. Nault reported that flow is estimated by taking readings from the community water supply's meter and subtracting 9000 gallons per day to account for the fact that roughly 30 residences are connected to the water system but not the sewer system. Wrnatnn-Salem_ NC 27105 "Influent" is defined in 15A NCAC 02B.0503(16) to mean "the wastewater entering a water pollution control facility." Further, "water pollution control facility" is defined to mean "treatment works", or, "any plant, septic tank disposal field, lagoon, pumping station, constructed drainage ditch or surface water intercepting_ ditch, incinerator, area devoted to sanitary landfill, or other works not specifically mentioned herein, installed for the purpose of treating, equalizing, neutralizing, stabilizing, or disposing of waste" (GS 143- 213(17)). Based on the above, please be advised that your current method of flow measurement does not comply with the requirement for continuous flow measurement. Note also that Part II, Section D.3 requires approval of the flow measurement device and monitoring location prior to installation, and that the definition of flow measurement can be found in Part 11, Section A. Documentation Review 6. Records/Reports Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs include all permit parameters. Copies of DMRs, operator visitation logs, lab reports and Chain -of -Custody forms were available for review during the inspection. 7. Self -Monitoring Program A review of DMRs from January, 2015 to April, 2017 revealed no violations. 8. Laboratory All parameters are analyzed by Meritech, Inc. DMRs were compared with lab documents and no errors or concerns were noted. 9. Sludge Handling Disposal Mr. Nault reported that you arrange for pumping of the septic tanks throughout the community and maintain those records. Records were not available during the inspection. Required Action - To prevent further action by this Division, within 30 days of receiving this report, please reply in writing with a timeline for installing a continuous flow measurement device that will comply with permit conditions.
