Subject: Request for a NWP 3 and 401 Water Quality Certification for B-5609, the replacement
of Br�idge #28 over Cutawhiskie Creek on SR 1137 in Hertford County. WBS # 45564.1.1
The North Carolina Departmei�t of Transportation is requesting the issuance of a
Nationwide Permit 3 and corresponding 401 Water Quality Certification for the replacement of
Bridge #28 in Hertford County. The bridge is located on SR 1137 Liverman Mill Road
approximately 0.53 miles west of SR 1139. The bridge crosses Cutawhiskie Creek, classified by
NCDWR as C;NSW waters. The bridge is currently a 154' long, 9-span timber bridge with
timber deck. This structure will be removed using NCDOT's guidelines for bridge removal. The
proposed bridge will be placed on its existing alignment. The new bridge sh•ucture will be a
three-span concrete cot•ed-slab bridge; 170'L X 34'W. Jurisdictional wetland impacts have been
minimized, resulting in 0.05 acres of permanent fill in wetlands and 0.05 acres of temporary
hand-clearing in wetlands. No permanent stream impacts are proposed. NCDOT will follow
stream crossing guidelines for anadromous fish passage, including an in-water• work moratoriuin
fi•om Febr�ualy 15 to June 30. An off-site detour will be utilized during construction.
