The two state entities that we'll be meeting with at 1:30pm today would like to have the
Owner agent form filled out & returned in order to log the project into their
database. (Not necessarily for today's meeting - just in general.).
It looks like something that would be more appropriate for you to sign than me, but it's
entirely up to you.
Let me know if you have any questions or would prefer to have me complete it.

Jennifer Burdette with DEQ asked that we go ahead and shoot her a
signed copy of the agent authorization form. Once received, they will go
ahead and log it into their system. The attached form will need to be
signed by a representative for the property owner.
