Lee Active Ash Pond Wayne County State Dam ID: WAYNE - 022 Dear Mr. Hamrick: ROY COOPER Governor MICHAEL S REGAN Secretary TRACY E DAVIS Director RECEIVEDUDEWWR FEB 1 6 2017 Water Quality Permitting Section This correspondence concerns the referenced high hazard dam repaired pursuant to issuance of an Approval of Repair dated October 13, 2015, as required by the Dam Safety Law of 1967. The repair documentation pertaining to Scope 1, Addendum 2 — (Resubmittal of Alternate spillway system) certified by Mr. Lester Lowe, PE, AMEC., was received by our office on March 23, 2016. An inspection of this dam was made by the Land Quality Section staff from the Washington Regional Office on April 15, 2016. The dam repair was found to be in general conformance with the approved plans and specifications, and continued operation of the dam is hereby approved. The Land Quality Section staff will make periodic inspections of this dam to verify that the dam is being maintained in good operating condition. It is advised that you closely inspect and monitor your dam, and that you notify your engineer and the Division of Energy, Mineral, and Land Resources if you notice or suspect any problems concerning its safety. The Emergency Action Plan must be reviewed and updated annually, or as needed by the owner or the owner's representative.
