Smith/Camp Spring 76 — Groundwater Remediation Site NPDES Permit No. NCO088692 Caswell County Dear Mr. Sparks: On June 05, 2017, Min Xiao and Paul DiMatteo of this office met with Danny Oldham, to perform a Compliance Evaluation Inspection on the petroleum contaminated groundwater remediation site. The inspection found the treatment system to be compliant with the NPDES permit. The inspection findings are detailed below and an inspection checklist is attached. I. Permit The permit has been expired on May 31, 2017. A renewal application and supporting documentation were prepared by Pyramid Environmental & Engineering, PC and received by the Water Quality Permitting Section on November 22, 2016. H. Records/Reports For this facility, the DMRs were reviewed for the period January 2016 through December 2016. The facility was found to be compliant with the effluent permit limits for the above period. Flow is measured onsite, other parameters are measured by Pace Analytical Labs. A comparison of February 2017 DMR with the field records and laboratory results revealed that the data had been correctly transcribed. An ORC visitation log and a copy of the permit were kept onsite, the DMRs and lab results are kept in the office of Pyramid in Greensboro. III. Facility Site Review The contaminated groundwater is treated with the following equipment: three recovery wells (each with an in-line totalizer), air compressor, particle filter, 150 -gallon EQ tank, oil -water separator (OWS), 150 -gallon OWS sump tank, air stripper exhaust, 150 -gallon sump tank, 4 activated carbon filters and 1 aggregate filter, an effluent totalizer. The facility appeared to be competently operated and maintained. Flow Measurement The permit requires continuous flow recording. Flow data reported on the DMR are obtained from mechanical totalizers. VI. Effluent/Receiving Waters The effluent flows through a PVC pipe with gravity and discharges into an unnamed tributary of Country Line Creek, class "WS -II; HQW" waters of the Roanoke River Basin.
