Per the Wilkes county tax map, you are the owner of the subject parcel, which has a single-family residence
dwelling. The Wilkes County Health Department has no records of a wastewater treatment system for your
single-family residence. It appears from our documents that the wastewater may discharge into the Wilkes
County Adult Care, Wastewater Treatment Plant (WWTP), NPDES Permit # NC0068543.
The Division of Water Resources is seeking a revocation of this permit, therefore, if your single-family
residence at 174 Rest Home Road discharges into this WWTP, then your wastewater discharge would
become an unpermitted discharge. Also, if the WWTP and the Care Facility are sold to another entity,
wastewater discharges into the WWTP will be subject to the discretion of the new owner.
There are two possibilities that you may pursue concerning the wastewater discharge from your single-
family residence:
1. Contact the Wilkes County Health Department at 336-651-7450 and request a site inspection for a
subsurface wastewater system.
2. Contact the Winston-Salem Regional Office to determine options for a permit with the North
Carolina Division of Water Resources.
