Section 401/404/CAMA permit(s) issued for the above referenced project through purchase of
credits from the Box Creek Wilderness Stream Mitigation Bank. The amount you owe is based

If a 10 percent deposit of the amount due is not received within 60 days of the date of this invoice, it will
expire. If the 10 percent deposit is received, the total amount of requested credits will be reserved for 120
days from the date of this invoice at the unit price listed in this invoice. The deposit is refundable if the
Corps of Engineers does not issue the 404 permit. If full payment is not received within 60 days of the date
of the issued DWR and Corps permits (whichever is later), then this mitigation invoice will expire. Note
that your permit(s) may require payment before this date. Subsequent invoice extensions will be based on
the fee schedule in effect on the date they are issued. This mitigation invoice/credit reservation will expire
after 120 days from the date of this invoice. Please send a check payable to: 130 of Chatham, LLC. for
the deposit amount to the address below and enclose a copy of this invoice. Once permits are in hand
please send a second check for the remainder of the amount due.
