A review of the December 2015 Discharge Monitoring Report (DMR) for the subject facility revealed the
deficiency(s) indicated below:

Please be aware that non-compliance with your permit could result in enforcement action by the Division of
Water Resources for these and any additional violations of State law. The Mooresville Regional Office
encourages you to take all necessary actions to bring your facility into compliance. It is noted that your current
NPDES Permit #NC0049867, Part I, Footnote: (7.) states that the "permittee shall perform 3 Effluent Pollutant
Scans during the term of the permit". In section A. item (3.) Effluent Pollutant Scan — states that "One scan must
be performed in each of the following years: 2015, 2016, and 2017."
Due to the scan for 2015 being missed, this Office grants permission to collect an additional Effluent Pollutant Scan
during the year 2018, to be in compliance with the required 3 Effluent Pollutant Scans required by the current
permit term. This Office encourages the permittee to closely review terms and conditions of the current NPDES
permit to avoid any further sampling frequency violations.
