Subject: REQUEST FOR ADDITIONAL INFORMATION
Shiloh Subdivision
Dear Mr. Gonzales:
On May 24, 2017, the Division of Water Resources — Water Quality Programs (Division) received your
application dated May 17, 2017, requesting a 401 Water Quality Certification from the Division for your
project. The Division has determined that your application is incomplete and cannot be processed. The
application is on -hold until all of the following information is received:
1. Please provide the Division a copy of your response to May 23, 2017 email from the US Army Corps
of Engineers requesting additional information. [15A NCAC 02H .0502(c)]
A complete review of the project cannot be conducted until the U.S. Army Corps of Engineers has
verified all stream and wetland locations. [15A NCAC 02H .0502(a)(7)].
As indicated under Item C.3.g (Page 13) of the USACE PCN Complete Help File "The stream impact
length should be measured along the centerline of the stream. When proposing a culvert, the impact
length is generally greater than the length of the culvert and associated dissipater since the existing
stream usually has some sinuosity (curvature)." Please verify or correct the stream impact length as
indicated on the PCN form. Please provide updated drawings and application materials (including
Table of Impacts in the PCN form) that properly show the length of impacts to the jurisdictional
stream. [15A NCAC 02H .0502(b)]
4. Please realign the outlet location of the 42" culvert for the road crossing so that it is in better
alignment with the stream channel. [15A NCAC 02H .0506(b)(2)]
5. Please note that Item E. 1a. of the PCN form is incorrect, this location drains to the Dan River,
Roanoke River Basin and therefore is not subject to any of the state riparian buffer protection rules.

Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information
for the proper consideration of the application. If all of the requested information is not received in writing
within 30 calendar days of receipt of this letter, the Division will be unable to approve the application and it
will be returned. The return of this project will necessitate reapplication to the Division for approval,
including a complete application package and the appropriate fee.

note that the DWR# that is requested on the link is referenced above.
Please be aware that you have no authorization under the Water Quality Certification Rules for this activity
and any work done within waters of the state may be a violation of North Carolina General Statutes and
Administrative Code.
