To correct the above -referenced deficiencies, you are to submit revised DMRs for the referenced
months within 14 days of receipt of this Notice. If a site visit was not performed, the DMR should accurately
reflect this information in accordance with DMR guidance documents. Note that temperature monitoring is required
5 times a week and therefore, the DMR must reflect either a measurement or a reason for not monitoring each of
the required 5 days in a week.
